Today I told a patient he has to refill his medical history form because I could not read it, he said no your refill it and I will tell you my symptoms. I have diarrhea and farted, an upset stomach and burped and than he said I have erectile dysfunction if you care to see. If you don't want to see I suggest you fill out that form yourself.
